To request a transcript, proof of enrollment or a grade report, please fill out the following electronic form. This electronic form is for High School Diploma and LVN requests ONLY. For High School Equivalency transcript request from GED or HiSET go to the next tab and follow the instructions.

Oxnard Adult School does not maintain transcripts, records and/or certificates of students who took the GED or HiSET exam. To request a certificate, a transcript or any other records please refer to the appropriate agency.

Transcript/Certificate Verifications

To request a document verification or replacement copy, only the test taker can initiate a CDE search. Federal law prohibits CDE staff from discussing test taker information with 3rd party requestors. Learn more...
